New clients: please complete the New Client Intake Form (Arizona) once, and the Telemedicine Consent & Today Visit (Arizona) form before your first telemedicine appointment.
Returning clients: you do not need to fill out the intake form again. For each new telemedicine visit, please only complete the Telemedicine Consent & Today Visit (Arizona) form.
